Creating a new questionnaire is possible via the corresponding button in the Comet tool:

First steps include setting the following:

• Title: Choose a title for your questionnaire.
• Subjects: Choose the competence grid that your self assessment will be based on. 
• Thumbnail: Optionally, choose a thumbnail picture for your assessment
